Adding HBS Transportation Services to Your Existing Enrollment

For companies already enrolled for Self-Direction Assistance or Personal Support-Agency Based services and wanting to add HBS Transportation Services to their service offerings, please follow these steps:

  1. IMPACT Provider Enrollment System Login: Begin by accessing the IMPACT Provider Enrollment System using your personal IMPACT account User ID and Password.
  2. Enrollment Modification Request: Utilize the IMPACT Enrollment Modification Request PowerPoint Presentation. This helpful resource is available on the IMPACT Presentations and Materials page of the HFS website. This presentation will guide you through completing the modification request.
    • Specialty Addition: Add “Transportation” as a specialty, and specify “Private Auto” as the subspecialty. Leave “No subspecialty” selected.
    • License/Certification/Other Information: Enter all required details, including the driver’s license information, vehicle identification numbers (VINs), vehicle registration details, and vehicle insurance information.
  3. DHS, DDD Provider Enrollment Notification: After submitting the IMPACT modification request, notify DHS, DDD Provider Enrollment by sending an email to [email protected]. This informs them that the request is submitted and ready for State Review.
  4. IMPACT Approval Notification: You will receive an email notification directly from the IMPACT system once your application has been reviewed and approved.
  5.
