Applying for PT-1 Transportation Services
If you are deemed eligible, the next step is to apply for services. You will need a Prescription for Transportation (PT-1) form. To obtain this form, you can:
- Contact MassHealth Customer Service: Call the toll-free number at 800-841-2900 or 800-497-4648 (TTY). You can request that a PT-1 form be faxed directly to your MassHealth medical provider.
- Provider Request: Your MassHealth medical provider can also request the PT-1 form on your behalf.
