Who is Eligible for MassHealth Transportation?

As a MassHealth member, you may qualify for non-emergency transportation benefits to and from MassHealth-covered services if you cannot use public or private transportation. These services include a range of healthcare needs, such as:

  • Doctor’s appointments
  • Counseling sessions
  • Day habilitation programs

Applying for PT-1 Transportation Services

If you are deemed eligible, the next step is to apply for services. You will need a Prescription for Transportation (PT-1) form. To obtain this form, you can:

  1. Contact MassHealth Customer Service: Call the toll-free number at 800-841-2900 or 800-497-4648 (TTY). You can request that a PT-1 form be faxed directly to your MassHealth medical provider.
  2. Provider Request: Your MassHealth medical provider can also request the PT-1 form on your behalf.

The PT-1 form must be completed and signed by your MassHealth medical provider. This form serves as a “prescription” for transportation, confirming your medical need for transportation assistance. The completed PT-1 form can be submitted online, faxed, or mailed to the CSC by your provider.

Once the CSC receives your PT-1 form, processing typically takes up to three business days. If transportation is needed more urgently, your medical provider can request a verbal authorization by contacting the CSC at 1-800-841-2900 (TTY: 1-800-497-4648).

PT-1 Approval, Denial, and Scheduling Rides

After submitting your PT-1 form, you will receive a notification by mail regarding its status.

  • Approval: If your PT-1 is approved, the letter will include instructions on how to arrange your transportation services.
  • Denial: If denied, the letter will explain the reason for denial and provide information on how to appeal the decision.

Upon PT-1 approval for MassHealth transportation, a transportation broker will be assigned to you. This broker will be your point of contact for arranging all your rides.
